What you will do:
- Conduct assessments and create personalised care plans, ensuring the safety and wellbeing of individuals and families.
- Collaborate with a multi-disciplinary team to provide support, interventions, and appropriate services to meet the needs of service users.
- Liaise with external agencies and community partners to coordinate resources and services, fostering strong community relationships.
- Maintain accurate and up-to-date records, ensuring compliance with relevant legislation and policies.
- Provide crisis intervention when required, demonstrating empathy and resilience in challenging situations.
- Attend and contribute to team meetings, training sessions, and development opportunities, encouraging professional growth and shared learning.
- Advise and support less experienced team members, sharing expertise to strengthen the overall service provision.
Our opportunities span both Children and Adult Services, with permanent roles available in various teams including Childcare Management, Adoption, and Family Placement within Children Services, as well as positions in Joint Discharge and within the North and South Teams for Adult Services. Work locations are either County Hall in Haverfordwest or Argyle Street in Pembroke Dock.
