What you will do

  • Community Assessments: Complete comprehensive occupational therapy assessments within community settings.
  • Rapid Response Interventions: Provide urgent interventions to support individuals in crisis and prevent unnecessary hospital admissions.
  • Hospital Discharge Support: Facilitate safe and effective hospital discharges, promoting independence and reducing delays.
  • Reablement Services: Deliver therapeutic interventions aimed at restoring functional ability and maximising independence.
  • Care Planning: Develop and review person-centred treatment and support plans tailored to individual needs.
  • A&E Support: Provide occupational therapy input for selected urgent cases presenting through emergency services.
  • Multi-Disciplinary Working: Collaborate with healthcare professionals to ensure coordinated and effective care delivery.
  • Record Keeping: Maintain accurate clinical records, treatment plans, and progress reports.
  • Outcome Monitoring: Evaluate intervention effectiveness and adapt care plans to achieve the best possible outcomes.

Qualifications / Requirements

  • Occupational Therapy Qualification: Degree in Occupational Therapy or equivalent recognised qualification.
  • Registration: Current HCPC registration as an Occupational Therapist.
  • Experience: Previous experience within community occupational therapy, rehabilitation, reablement, or discharge planning services.
  • Assessment Skills: Strong clinical assessment and care planning abilities.
  • Community Practice: Experience supporting individuals in community settings and promoting independent living.
  • Communication Skills: Excellent interpersonal, written, and verbal communication skills.
  • Teamwork: Ability to work effectively within multidisciplinary teams.
  • Driving Licence: Full UK driving licence is desirable due to the community-based nature of the role.
