What You Will Do:

- Manage a varied and complex caseload while receiving guidance from the Lead Practitioner, ensuring accountability and professional practice.
- Conduct comprehensive needs assessments in line with the Care Act 2014, promoting independence and identifying viable support solutions.
- Engage with specialist safeguarding colleagues, contribute to safeguarding enquiries, and focus on the individual’s desired outcomes.
- Act as a deputy for the Lead Practitioner when required, attending multi-disciplinary meetings and representing the team.
- Mentor and supervise both non-registered social care practitioners and social work students, fostering a culture of support and professional development.
- Develop value-for-money care packages in collaboration with individuals and caregivers, meeting eligibility criteria and maximizing quality of life.
- Work closely with individuals, families, and communities to facilitate informed decisions, contributing to effective service planning.
- Keep abreast of available resources and services, identifying gaps in provision, and operating within delegated budget mechanisms.
