This outstanding opportunity invites you to step into a key role within the Adults Discharge to Assess Team in Nottingham. Working 37 hours per week with a minimum of three days face-to-face, your day will start at 9:00 and end at 17:00. What you will do:
- Conduct comprehensive assessments of individuals discharged from hospital to determine their care needs.
- Collaborate with multidisciplinary teams to develop and implement care plans that promote successful transitions and ensure adequate support.
- Advocate for service users to ensure access to appropriate resources and services within the community.
- Maintain accurate and up-to-date records of assessments, care plans, and other relevant documentation.
- Communicate effectively with patients, families, and carers, providing guidance and support as needed.
- Attend meetings and contribute to case discussions, ensuring that all actions align with best practices and current legislation.
Possession of a full driving licence, access to a vehicle, and willingness to travel both inside and outside the council area as required is necessary. The role requires an experienced social worker who is adept at assessment work, with in-depth knowledge of Discharge to Assess (D2A) processes, hospital work, and strong multi-agency collaboration skills. Knowledge of Continuing Healthcare (CHC) is also vital.
