What you will do

• Manage a complex caseload involving children with disabilities, long-term health conditions, and additional care and support needs.
• Undertake statutory assessments, child and family assessments, care planning, and reviews in accordance with legislative requirements.
• Lead safeguarding investigations and contribute to Child in Need, Child Protection, and Looked After Children processes where appropriate.
• Coordinate multi-agency support involving health professionals, education providers, SEND services, early help teams, and commissioned care providers.
