Overview

An exciting opportunity has arisen for an experienced Senior Social Worker to join the Adult Locality Team in Medway. This role is ideal for a confident practitioner with specialist knowledge of NHS Continuing Healthcare (CHC), who is looking to take on a leadership role within a supportive and forward-thinking service. Main Purpose of the Role

As a Senior Specialist CHC Assessor, you will lead on Decision Support Tool (DST) assessments to determine eligibility for NHS Continuing Healthcare funding. You will manage complex and contested cases, including joint funding arrangements, while supporting individuals and families throughout the assessment process.

You will work closely with internal teams, NHS colleagues, care providers, and local authority partners to ensure appropriate care and support packages are implemented. The role also includes undertaking Section 117 reviews, contributing to service development, and providing professional supervision to a small number of social care staff to promote high standards of practice.


Key Responsibilities

  • Lead and coordinate CHC assessments and reviews, both face-to-face and virtual

  • Manage a complex caseload from assessment through to resolution

  • Provide specialist advice on CHC eligibility, Section 117 aftercare, and joint funding pathways

  • Work collaboratively with partners to arrange care packages and placements

  • Support individuals and families with clear, empathetic communication throughout the CHC process

  • Maintain accurate and timely records in line with statutory and organisational requirements

  • Contribute to performance monitoring, data reporting, and governance processes

  • Support staff development through supervision, training, and induction

  • Contribute to service improvement and continuous development initiatives


Essential Criteria

  • Degree or equivalent qualification in Social Work

  • Registration with Social Work England

  • Significant post-qualifying experience within adult social care

  • Strong working knowledge of NHS Continuing Healthcare, DSTs, and Section 117

  • Experience managing complex cases and working across health and social care systems

  • Ability to supervise and support social care staff

  • Strong communication, analytical, and decision-making skills
