Role Purpose

The Qualified Social Worker within the Adults CHC Team plays a vital role in assessing, coordinating, and supporting individuals through the Continuing Healthcare process. The role ensures that adults with complex health and social care needs receive appropriate, person-centred support that promotes dignity, wellbeing, and independence.

What You Will Do

Undertake strengths-based assessments in line with the Care Act 2014, clearly identifying needs and outcomes.
Participate in Continuing Healthcare assessments alongside health professionals, including completion of Decision Support Tools (DSTs).
Support individuals and families through the CHC process, providing clear explanations of eligibility decisions and outcomes.
Develop, review, and monitor person-centred care and support plans to ensure they remain proportionate and outcome-focused.
Work collaboratively with NHS Integrated Care Boards, clinicians, care providers, and other professionals across the CHC pathway.
Manage a caseload of adults with complex and high-level needs, coordinating reviews and multi-disciplinary meetings.
Identify and respond appropriately to safeguarding concerns in line with statutory duties and local procedures.
Maintain accurate, timely records and ensure compliance with policy, legislation, and quality standards.

Person Specification

Qualified Social Worker with current Social Work England registration.
Strong working knowledge of the Care Act 2014, Mental Capacity Act 2005, and safeguarding adults legislation.
Experience working within adult social care, ideally with exposure to CHC or complex needs pathways.
Ability to manage complex caseloads and work confidently within multi-agency environments.
Excellent communication, organisational, and decision-making skills.
