What you will do:

• Undertake comprehensive Occupational Therapy assessments for adults with physical disabilities, frailty, long-term conditions, and complex needs.
• Develop, implement, monitor, and review goal-focused reablement plans that maximise independence in activities of daily living.
• Assess the need for specialist equipment, assistive technology, adaptations, and moving and handling solutions.
• Work collaboratively with service users, carers, social workers, physiotherapists, reablement support workers, health professionals, and partner agencies.
• Provide advice, guidance, and training to clients, carers, and colleagues on rehabilitation and reablement techniques.
• Complete strengths-based assessments in accordance with the Care Act 2014 and relevant legislation.
• Promote independence and reduce reliance on long-term care packages wherever appropriate.
• Maintain accurate, timely, and professional case records and reports.
• Participate in safeguarding investigations and risk management processes where required.
• Contribute to service development, quality improvement initiatives, and best practice within Adult Social Care.

Requirements:

• Degree in Occupational Therapy or equivalent qualification.
• Registration with the Health and Care Professions Council (HCPC).
• Experience working within reablement, rehabilitation, or adult social care settings.
• Ability to undertake comprehensive assessments and develop goal-focused reablement plans.
• Strong knowledge of the Care Act 2014 and relevant legislation.
• Excellent communication and multidisciplinary working skills.
• Ability to assess and recommend specialist equipment, adaptations, and assistive technology.
• Car driver essential.
