What you will do:
- Undertake comprehensive Occupational Therapy assessments for adults with physical disabilities, frailty, long-term conditions, and complex needs.
- Develop, implement, monitor, and review goal-focused reablement plans that maximise independence in activities of daily living.
- Assess the need for specialist equipment, assistive technology, adaptations, and moving and handling solutions.
- Work collaboratively with service users, carers, social workers, physiotherapists, reablement support workers, health professionals, and partner agencies.
- Provide advice, guidance, and training to clients, carers, and colleagues on rehabilitation and reablement techniques.
- Complete strengths-based assessments in accordance with the Care Act 2014 and relevant legislation.
- Promote independence and reduce reliance on long-term care packages wherever appropriate.
- Maintain accurate, timely, and professional case records and reports.
- Participate in safeguarding investigations and risk management processes where required.
- Contribute to service development, quality improvement initiatives, and best practice within Adult Social Care.
Requirements:
- Degree in Occupational Therapy or equivalent qualification.
- Registration with the Health and Care Professions Council (HCPC).
- Experience working within reablement, rehabilitation, or adult social care settings.
- Ability to undertake comprehensive assessments and develop goal-focused reablement plans.
- Strong knowledge of the Care Act 2014 and relevant legislation.
- Excellent communication and multidisciplinary working skills.
- Ability to assess and recommend specialist equipment, adaptations, and assistive technology.
- Car driver essential.
