What you will do

• Provide reablement services for 16 beds, ensuring smooth hospital discharges and supporting community step-up admissions.
• Collaborate with a multidisciplinary team to develop personalised plans that promote independence and help prevent future hospital admissions.
• Assess and evaluate patient needs, adapting rehabilitation programmes as required to optimise recovery and reablement outcomes.
• Utilise your HCPC registration and expertise to deliver high-quality patient care.
• Travel to various locations as required, as a car driver is essential for this role.
