What You Will Do:

• Conduct holistic assessments to identify individual needs and create tailored care plans.
• Recommend and arrange adaptations, including minor and major home modifications, to support independent living.
• Advise on postural seating, moving and handling, and provide equipment to enhance daily functioning.
• Offer rehousing recommendations for clients requiring more suitable accommodation.
• Work collaboratively with a dynamic multidisciplinary team to deliver efficient, person-centred support.

Requirements:

• HCPC registration as an Occupational Therapist.
• Full UK driving licence, with the ability to travel to community visits.
