What you will do:

- Conduct comprehensive housing needs and risk assessments on prison wings or via remote link to identify prisoners' housing histories and support needs.
- Work closely with Prison Offender Managers and Community Probation Officers to design customised, person-centred resettlement action plans that align with licence and supervision requirements.
- Source and secure suitable housing options including Private Rented Sector accommodation, Houses in Multiple Occupation, supported housing projects, and Approved Premises.
- Deliver seamless "through-the-gate" support, ensuring each client is aware of their post-release accommodation and providing assistance with immediate physical check-ins.
- Establish relationships with private landlords, local authorities, and Registered Social Landlords, advocating on behalf of clients with criminal convictions.
- Manage welfare and grant applications such as setting up benefit claims including Universal Credit housing element, and applying for discretionary housing grants for initial expenses.
