What you will do:

- Engage with individuals to conduct needs assessments and contribute to crafting care plans in line with the Care Act 2014.
- Play an essential role in safeguarding procedures, identifying and addressing risks under the Safeguarding Vulnerable Groups Act 2006.
- Coordinate and monitor care packages, liaising with multidisciplinary professionals such as GPs, nurses, and social workers.
- Champion independence and person-centred care, promoting well-being for service users.
- Maintain detailed and accurate records, ensuring compliance with standards set by the Care Quality Commission.
- Provide both practical help and emotional support to individuals and their families/carers.
- Respond effectively to changing needs, including managing crisis or emergency situations.
