You will work alongside families and professionals to create, implement, monitor, and amend Education, Health, and Care Plans in alignment with the Children and Families Act 2014 and the SEND Code of Practice 2014.

What you will do:
- Coordinate the EHCP process by producing high-quality plans, consulting with multidisciplinary professionals, educational settings, and families to ensure the plans reflect the best interest of the child or young person.
- Manage a caseload of EHCPs, providing families with a dedicated point of contact within the local authority and approving funding allocations.
- Represent the SEND service at meetings and panels, ensuring the educational provision continues to meet the evolving needs of the child or young person.
- Collaborate with Education and Skills, Children's and Adult's social care services, Health, and other agencies to support the writing and amendment of EHCPs.
- Prepare and present necessary documentation at multi-agency placement panels, and stay informed about changes in legislation, policies, and procedures.
Please note: You may at times be required to work outside of core office hours to fulfil the responsibilities of this role. A DBS Check is an essential requirement for working with children and young people.
