What you will do:

Coordinate care provision with multidisciplinary and multi-agency teams.
Arrange and attend complex meetings relating to service user care and treatment plans.
Review treatment plans regularly and facilitate appropriate transitions to primary care or other services.
Manage your own caseload effectively with regular supervision and support.
Undertake risk assessments and apply risk management strategies within mental health care settings.
Participate in the Duty rota to ensure responsive and comprehensive care delivery.
Work autonomously while contributing proactively to a service user-centred care model.

Requirements:

HCPC registration as an Occupational Therapist.
Previous experience within mental health services is essential.
Strong communication, assessment, and multidisciplinary working skills.
