Band 6 Occupational Therapist within the Acute Medicine Team in Hammersmith, earning £26.00 per hour. What you will do:

• Provide specialist occupational therapy assessment and intervention within the Acute Medicine Team.

• Work across renal and specialist medicine wards, delivering personalised rehabilitation and discharge planning interventions.

• Assess patients' functional abilities, mobility, cognition, and discharge needs to support safe transitions from hospital.

• Independently manage discharge planning processes, ensuring timely and effective patient flow.

• Prioritise and manage a varied caseload within a fast-paced acute hospital environment.

• Recommend equipment, adaptations, and support services to maximise patient independence and safety.

• Work closely with medical, nursing, physiotherapy, social care, and discharge planning teams to achieve positive outcomes.

• Support patients and families through education, advice, and rehabilitation planning.

• Maintain accurate, timely, and comprehensive clinical documentation in line with professional and organisational standards.

• Contribute to service development, quality improvement initiatives, and best practice within the department.

Requirements:

• Current HCPC registration as an Occupational Therapist.

• Previous experience working at Band 6 level or equivalent within an acute hospital setting.

• Strong experience within acute medicine services.

• Neurological rehabilitation experience is desirable.

• Excellent discharge planning and caseload management skills.

• Ability to work independently and effectively within a multidisciplinary team.

• Strong communication, assessment, and clinical reasoning abilities.
