What you will do

Care Coordination: Manage a caseload of adults requiring mental health support and coordinate person-centred care plans.
Assessment: Undertake comprehensive mental health assessments and identify individual needs and risks.
Care Planning: Develop, implement, and review personalised care plans in collaboration with service users and multidisciplinary teams.
Recovery-Focused Practice: Support individuals with moderate to severe mental health needs, including psychosis, to achieve recovery goals.
Multi-Agency Working: Work closely with psychiatrists, psychologists, social workers, GPs, and other healthcare professionals.
Risk Management: Assess, monitor, and manage risk while promoting safety and wellbeing.
Legislative Compliance: Apply knowledge of the Mental Health Act, Care Programme Approach (CPA), and relevant legislation within practice.
Crisis Support: Respond appropriately to complex and distressing situations while maintaining professional judgement and resilience.
Record Keeping: Maintain accurate and timely clinical records using electronic systems and digital tools.
Communication: Prepare reports, care documentation, and professional correspondence using Microsoft Word, Excel, and PowerPoint.

Qualifications / Requirements

Professional Registration: Current NMC registration as a Registered Mental Health Nurse (RMN).
Experience: Minimum of one year's post-registration experience within mental health services.
Community Experience: Previous experience within community mental health services is highly desirable.
Mental Health Knowledge: Strong understanding of psychosis, severe mental illness, recovery-focused practice, and risk management.
Legislative Knowledge: Working knowledge of the Mental Health Act, Care Programme Approach, and relevant mental health legislation.
Communication Skills: Excellent verbal, written, and interpersonal communication abilities.
IT Skills: Proficiency in Microsoft Word, Excel, PowerPoint, and electronic case management systems.
Driving Requirement: Full UK driving licence and access to a vehicle are essential.
