About the role

We are seeking a motivated and compassionate Band 6 Community Psychiatric Nurse (CPN) to join the Community Mental Health Team for Older People (CMHTOP) in Surrey. Job purpose

To deliver high-quality mental health nursing care to older adults with complex mental health needs, promoting recovery, maintaining independence, and enhancing overall wellbeing within a community setting.


Key responsibilities

  • Undertake comprehensive mental health, risk, and capacity assessments for older adults.

  • Manage a defined community caseload, acting as Care Coordinator under the Care Programme Approach (CPA).

  • Develop, implement, and regularly review person-centred care plans in partnership with service users, carers, and professionals.

  • Deliver evidence-based nursing interventions, including medication management and physical health monitoring.

  • Administer depot medication and support treatment adherence.

  • Identify and manage safeguarding concerns in line with Trust and local authority procedures.

  • Work collaboratively within a multidisciplinary team, including psychiatrists, psychologists, social workers, and occupational therapists.

  • Liaise effectively with GPs, primary care services, voluntary agencies, and other stakeholders.

  • Participate in MDT meetings, case reviews, and discharge planning.

  • Maintain accurate, timely, and confidential electronic clinical records.

  • Ensure practice complies with the NMC Code, Trust policies, and relevant legislation.

  • Contribute to clinical audits, service evaluation, and quality improvement initiatives.

  • Provide guidance, supervision, and support to junior staff and students, acting as a professional role model.


Person specification

Essential criteria

  • Registered Mental Health Nurse (RMN) with current NMC registration.

  • Post-registration experience in mental health nursing, ideally within community services.

  • Knowledge of mental health conditions affecting older adults, including dementia.

  • Experience in risk assessment, care planning, and CPA processes.

  • Ability to manage complex caseloads autonomously.

  • Excellent communication, interpersonal, and organisational skills.

  • Basic IT skills for electronic patient record systems.

  • Full UK driving licence with access to a vehicle for community-based work.

Desirable criteria

  • Experience within Older Adult Mental Health services.

  • Knowledge of the Mental Health Act, Mental Capacity Act, and safeguarding legislation.

  • Experience supervising junior staff or students.

  • Non-medical prescribing qualification or willingness to undertake training.
