What you will do:
- Hold a diverse caseload, engaging with various adult social care statutory functions.
- Undertake comprehensive assessments under the Care Act and S117 Mental Health Act.
- Facilitate safe and effective hospital discharges.
- Conduct statutory safeguarding of adults, including concerns, enquiries, and case conferences.
- Participate actively in duty and social circumstance report rotas.
- Collaborate closely with co-located colleagues from the Pennine Care NHS Foundation Trust.
- Review services, form care plans, and ensure the best interests of adults in complex decisions.
- Lead mental capacity assessments and best interest meetings.
- Demonstrate a robust understanding of adult social care legislation.
- Possess a valid driving licence for work purposes and ability to work predominantly in the office with some flexibility for remote work.

Desirables:
- Previous experience with social circumstance reports and Court of Protection work, including S21A challenges.

As an experienced Adult Care Social Worker, you will bring a minimum of 3 years of relevant experience, a registration with Social Work England, and a satisfactory DBS check to ensure high standards of care.
