Locum Adults Social Worker within Localities in Kingston upon Thames, earning £32 Hourly, Ongoing

This is an exciting opportunity for a skilled Adults Social Worker to join our Close-Knit Team in Kingston upon Thames. You will be part of the Adult Social Care service within the New Malden Hub, one of three locality teams, focused on supporting adults with physical health and social care needs. The role is based at the Guildhall 2 two days a week, providing an engaging environment for team meetings and networking. Your primary clientele will include older adults and individuals with physical disabilities. What you will do:
- Facilitate effective assessment and support planning for adults with physical health needs in compliance with the Care Act 2014 and the Mental Capacity Act 2005.
- Manage and review service users’ cases, ensuring their needs are supported within statutory timescales.
- Undertake safeguarding investigations and care manage cases with varying levels of complexity and risk.
- Collaborate with colleagues within health and social care to ensure person-centred multidisciplinary approaches.
- Make referrals to appropriate services and manage resources effectively using strength-based care planning.
- Engage and contribute to team morale, participating in peer reviews and case discussions to bolster learning and development.

About You:
To excel in this role, you must hold an undergraduate or postgraduate degree in Social Work, be registered with Social Work England, and have the legal right to work in the UK. Excellent communication, assessment, and case management skills are essential. A thorough knowledge of personal budgets, self-directed support, relevant legislation, and safeguarding processes is also required.
