What you will do:

  • Undertake Care Act assessments, reviews, and support planning for adults with care and support needs.
  • Manage and prioritise a caseload while ensuring timely interventions, reviews, and meticulous record-keeping.
  • Develop outcome-focused support plans that emphasise independence, choice, and wellbeing.
  • Coordinate and monitor commissioned services to ensure care packages remain effective and tailored to individual needs.

Roles and Responsibilities:

  • Undertake assessments and develop support plans for adults with eligible care needs, aligning with the Care Act.
  • Support people in setting and achieving their personal goals, refining care packages as necessary.
  • Collaborate with colleagues and external partners to coordinate essential services.

Person Specification:

Qualifications:

  • NVQ Level 3 in Health and Social Care, Adult Care, or equivalent relevant qualification, or demonstrable equivalent experience.
  • A commitment to continuous professional development.
  • Having relevant training in safeguarding adults and risk assessment is advantageous.

Experience:

  • Experience working within Adult Social Care, community care, health services or a related public sector environment.
  • Practical experience in undertaking assessments and developing support plans for vulnerable adults.
