In this role, you will focus on supporting young people aged 14 to 25 with care needs, disabilities, and complex transitions into adulthood. Your work will aim to promote independence, inclusion, and positive long-term outcomes by collaborating closely with young people, their families, and a network of professionals across Children's Services, Education, Health, and SEND partners.

What you will do:

• Conduct assessments of young people's needs and support their transition to adult services.
• Develop care plans that reflect a strengths-based and person-centred approach.

• Advocate for the young people's needs, ensuring their voices are heard and considered in decision-making processes.
