What you will do:
- Efficiently assess patients’ needs within the Discharge2Assess framework, ensuring seamless transition from hospital to home.
- Develop comprehensive care plans tailored to each individual patients’ rehabilitation needs and realistic goals.
- Collaborate closely with multidisciplinary teams to ensure coordinated and optimal patient care.
- Monitor and evaluate patient progress, staying agile to adapt care plans in response to changing needs.
- Deliver high standards of care which positively influence patient independence and quality of life.
Requirements of the Occupational Therapist:
- Hold a degree or equivalent in Occupational Therapy with recent UK experience in a similar role.
- Demonstrate recent experience working within the NHS.
- Be registered with the HCPC and ready to bring your expertise to our dynamic team.
