What you will do:
- Assess and evaluate clients' needs within a housing context, prioritising their safety and independence.
- Develop personalised care and rehabilitation plans to support clients in adapting to their living environments.
- Liaise with housing teams, healthcare professionals, and social workers to ensure a cohesive approach to client care.
- Facilitate equipment provision and home modifications to ensure clients can live comfortably and safely.
- Monitor and review clients' progress, making necessary adjustments to intervention plans as required.
Requirements for the Occupational Therapist:
- A degree or equivalent qualification in Occupational Therapy.
- Recent experience working as an Occupational Therapist in the UK, with familiarity in NHS settings.
- Registration with the HCPC to practice as an Occupational Therapist in the UK.
