This is an exciting opportunity for a Social Worker to join our Hospital Discharge Team in Dudley, working a full-time post of 37 hours per week. This locum role offers a competitive hourly rate of £32 and is available for 6 to 18 months. What you will do:

  • Conduct Care Act assessments and initiate the process for recommendations
  • Perform capacity assessments as required
  • Attend and actively participate in ward reviews, pre-discharge meetings, and Section 117 reviews
  • Liaise regularly with housing providers and external support agencies to facilitate successful discharges
  • Meet regularly with the health discharge coordinator to ensure alignment in discharge planning
  • Act as a contact point to facilitate smooth social care support processes and escalate if necessary
  • Communicate effectively with locality adult social care teams
  • Explore respite and step-down options for service users
  • Attend funding panels, make recommendations, and present cases confidently
  • Support applications for Section 117 funding tools as needed
  • Foster strong working relationships between health and social care organisations involved in service user support
