What you will do:

• Provide a professional social work service to adults, their families, and carers in alignment with the Care Act 2014.

• Engage with individuals, families, groups, and communities to assess needs and intervene effectively, promoting choice, control and independence.

• Independently manage complex situations, assessing and managing higher levels of risk, and balancing support with control.

• Develop support plans with individuals, carers and families, using a strength and asset-based approach.

• Encourage wellbeing and independence to prevent or delay the deterioration of skills.

• Facilitate clear understanding of NHS Continuing Health Care processes and support when needed.

• Ensure compliance with legislation in advocacy services and mental capacity assessments.

• Promote a culture of learning and development through supervision and guidance of less experienced social workers.

• Train as a Practice Educator or AMCP to support statutory duties as required.
