What you will do:

• Conduct strength-based assessments to understand and support individuals' unique needs and capabilities.

• Develop person-centered support plans that empower service users to live as independently as possible.

• Arrange and coordinate long-term support arrangements tailored to each individual.

• Undertake Mental Capacity Assessments and engage in Court of Protection work as required.

• Manage an allocated caseload with precision, ensuring the delivery of high-quality social work services.

• Collaborate with service users, carers, and families to implement effective brokerage services and support solutions.
