What You Will Do:

• Conduct thorough assessments to identify functional limitations and determine individual care needs

• Deliver services according to the Care Act 2014 legislation ensuring comprehensive care act reviews are completed

• Collaborate effectively within a multidisciplinary team to ensure the best outcomes for clients

• Apply your strong knowledge of safeguarding principles and the Mental Capacity Act to safeguard vulnerable adults

• Develop creative interventions tailored to the unique needs of each individual

• Communicate clearly and effectively with clients, families, and colleagues to facilitate excellent care

• Manage your caseload efficiently using strong organisational and time management skills
