What you will do:

  • Deliver high-quality interventions following AfC’s assessment, planning, and intervention framework.
  • Conduct comprehensive whole-family assessments, maintaining professional boundaries and establishing realistic goals.
  • Coordinate and deliver targeted support for families, emphasising the well-being of children and young people in decisions.
  • Lead in priority areas such as Domestic Violence, Parental Mental Health, Housing, or SEND.
  • Identify and work with multi-agency partnerships to develop SMART plans in collaboration with families.
  • Record all assessments and interventions meticulously on case management systems.
  • Respond to significant events promptly and escalate matters appropriately to management.
  • Sustain positive changes, increased independence and reduced risk factors for families.
  • Promote a culture of partnership and innovation within the team and service.
  • To thrive in this position, you must have a Degree or equivalent in Social Work, a current Social Work England registration, and substantial frontline experience in social services within the UK.
