What you will do:

- Conduct care act assessments and timely reviews for citizens discharged from the hospital.
- Complete Mental Capacity assessments and facilitate best interest decisions.
- Support safeguarding procedures with a critical and knowledgeable approach.
- Review packages of care implemented by the hospital team and assess long-term support needs.
- Work collaboratively with City Council colleagues, partner agencies, and stakeholders to manage and plan present and future risk situations.
- Develop and supervise students and trainees, contributing to the development of staff.

Key Role Accountabilities:

- Build and manage relationships with service users and their families in line with statutory obligations.
- Assess and plan service users' individual needs for positive outcomes.
- Develop effective solutions, engage with stakeholders, and maintain accurate records.
- Manage resources to ensure service delivery improvement and engage in diversity initiatives positively.

Role Portfolio:

Work as a part of the Discharge to Assess Team to support the safe and timely transfer of care. Collaborate with health colleagues across hospital settings, intermediate care units, and the community to promote independence and ensure a strength-based approach. Take accountability for complex decision making in a diverse and ever-challenging environment.

Technical Requirements:

- Social Work Qualification
- Registration with the Health Care Professionals Council (HCPC)
- Willingness to consent to an enhanced DBS check
- Ability and willingness to travel within the City of Manchester
