What you will do:
- Collaborate within a fast-paced dynamic team to support discharges from acute and community hospitals.
- Work to prevent unnecessary admissions to hospitals by providing effective occupational therapy interventions.
- Conduct comprehensive assessments and develop tailored reablement plans for individuals you serve.
- Serve as a key communicator between multidisciplinary teams and community resources, ensuring seamless service delivery.
- Utilize your expertise to adapt environments, train carers and patients, and facilitate safe and efficient discharges.
Requirements of the Occupational Therapist:
- Degree or equivalent in Occupational Therapy
- Recent UK experience working as an Occupational Therapist
- Recent NHS experience
- Registered with the HCPC
- Possess community and rehabilitation experience as a minimum
- Car driver; being adaptable and willing to travel as required to deliver services where needed
