What You Will Do:

• Conduct Care Act assessments and reviews to develop and implement effective, person-centred care plans.

• Perform Mental Capacity Assessments and Best Interest Decisions to safeguard adults’ rights and welfare.

• Apply a strength-based approach, enabling individuals to regain independence and resilience within the community.

• Support smooth and safe hospital discharges, coordinating with healthcare professionals and carers.

• Engage in safeguarding procedures, ensuring vulnerable adults are protected and supported appropriately.
