We’re excited to offer a fantastic locum opportunity for an experienced Social Worker to join the Adult Discharge to Assess service in Bristol. This full-time, ongoing role pays £32 per hour and plays a crucial part in supporting adults as they transition safely from hospital care to the community, empowering them to regain independence with dignity and support.

What You Will Do:

Conduct timely assessments for adults being discharged from hospital settings.

Create and implement person-centred care plans tailored to individuals’ unique needs and goals.

Work closely with healthcare teams, community services, and families to coordinate safe, effective discharges.

Monitor progress, reviewing and adjusting care plans to support long-term well-being.
