What you will do:

• Conduct assessments to understand and support the needs of adults within the South Neighbourhood Team
• Deliver comprehensive Care Act duties to ensure the wellbeing and autonomy of service users
• Collaborate effectively with colleagues and external partners to provide holistic care and support
• Utilise your experience in Adult Social Care and Liquid Logic systems to maintain detailed and accurate records
• Facilitate and coordinate care plans, ensuring they are tailored to the individual's unique needs
• Advocate for individuals, ensuring they have a voice within the care system
