What you will do:

• Implement and maintain the principles and practices of the Care Act 2014, ensuring professional standards align with those set by Social Work England.
• Apply a strengths-based approach to work directly with individuals, families and carers, empowering them to achieve the best possible outcomes.
• Conduct effective assessments of risks and needs, ensuring transparent communication of care plans and support.
• Manage a caseload efficiently while ensuring high-quality care is provided through consistent support and expert guidance.
• Formulate personalized, integrated care plans that maintain individuals' independence and focus on attaining their desired outcomes.
• Collaborate seamlessly with multidisciplinary teams, residents and their families, while promoting a holistic approach to care.
• Ensure safeguarding frameworks are adhered to, protecting adults at risk and liaising with relevant agencies.
