What You Will Do

• Deliver expert physiotherapy assessments, treatments, and discharge plans for patients across busy acute and elderly care wards.

• Coordinate closely with multidisciplinary teams to ensure smooth transitions of care and optimal patient outcomes.

• Provide tailored rehabilitation to elderly patients, promoting independence and improving quality of life.

• Maintain comprehensive and accurate documentation aligned with professional standards and hospital policies.

Requirements

• HCPC registered Physiotherapist.

• Minimum 2 years' experience in acute medical and/or elderly care settings.

• Confident in working independently within a hospital team.

• Car driver preferred, due to potential movement between hospital sites.
