What you will do:
- Provide practical, emotional, and recovery-focused support to adults and young people experiencing mental health problems or learning disabilities.
- Build positive, trusting relationships with service users to understand their needs and support their recovery journey.
- Work collaboratively with families, friends, and other professionals such as mental health nurses, psychiatrists, social workers, and external agencies including housing, police, and training providers.
- Support individuals in managing challenges such as homelessness, substance misuse, social exclusion, unemployment, and basic skills deficits.
Core Responsibilities:
- Conduct risk assessments and develop personalised recovery support plans that identify goals, aspirations, and interventions tailored to individual needs.
- Plan and coordinate programmes of support, including move-on plans and structured routines to aid recovery and independence.
- Provide one-to-one keyworking sessions focusing on mental health awareness, symptom management, medication adherence, and resilience building.
- Facilitate group activities and workshops to promote engagement, social inclusion, and skill development.
- Offer emotional guidance and support to individuals in crisis, helping them to manage their mental health and wellbeing effectively.
- Assist in crisis intervention and use de-escalation techniques when necessary.
- Maintain accurate and up-to-date records including daily progress notes, risk management and safety plans, medication administration logs, and support plans.
- Write reports and participate in clinical reviews and team meetings to contribute to ongoing care planning and service improvement.
- Support clients with daily living skills such as cooking, cleaning, shopping, and maintaining their accommodation to promote independence and tenancy sustainment.
- Help clients access community resources, benefits, and other support services.
- Participate in supervision, reflective practice, and training to maintain and enhance professional skills and knowledge.
- Assist in supervising volunteers or students when required.
- Stay informed about current mental health legislation and sector developments.
- Encourage service users to build positive emotions, self-pride, and regain control over their lives to support long-term recovery.
- Raise community awareness about mental health issues to reduce stigma and foster acceptance.
