Your Key Responsibilities

• Conduct blood sampling and ensure timely analysis for clozapine monitoring.

• Interpret and act on blood results, escalating any clinical concerns within the MDT.

• Coordinate care with GPs, pharmacies, and other relevant health and social care professionals.

• Maintain strict adherence to infection control and safety protocols within the clinic.

• Foster effective communication with patients, carers, and team members, upholding respect and dignity.

• Supervise and mentor student nurses to support professional growth and knowledge transfer.

• Keep accurate records using RIO and EPMA clinical systems.

• Monitor clinic workflows and promptly address operational issues.

• Contribute to policy development and continuous quality improvement.

Reporting Structure and Location

• Reporting to: Clinical Manager

• Responsible to: Community Team Manager

• Base: North East Merton Integrated Recovery Hub, The Wilson Hospital, Mitcham

• Collaborations: GPs, community pharmacies, supported accommodation, voluntary sector services, and inpatient units.
