We are seeking three agency-transitions social workers for an initial three-month period to replace staff currently engaged with Consultancy Plus. This role is crucial in ensuring a seamless transition for young people moving from child to adult services—fulfilling our statutory responsibilities and providing essential support.
What you will do:
- Facilitate the transition process for young individuals as they move from child to adult services, ensuring a coordinated approach.
