What you will do:
- Lead chronic disease management, providing direct care and guidance for conditions like diabetes, COPD, and hypertension.
- Deliver advanced clinical assessments and offer health promotion advice emphasizing prevention and wellness.
- Support audit processes, champion clinical governance, and drive continuous service improvements.
- Act as a mentor to junior nurses and Healthcare Assistants, fostering a culture of learning and excellence.
- Liaise with GPs, pharmacists, social care, and other PCN staff to ensure seamless continuity of care.
- Conduct complex mental health assessments and lead crisis interventions and care planning.
- Supervise and nurture development in junior nurses, students, and support workers, ensuring high standards of care.
- Contribute to clinical governance, participate in incident reviews, and lead quality improvement initiatives.
- Support Mental Health Act/Mental Capacity Act responsibilities, ensuring safeguarding procedures and effective liaison with families/carers.
- Chair multidisciplinary team or review meetings to facilitate comprehensive and collaborative care approaches.
