What you will do:
- Conduct assessments in clients’ homes to identify individual needs and create customised care plans

- Offer expert advice and support to clients and their families
- Maintain accurate records and documentation in line with HCPC standards
- Travel throughout the Medway area using your own vehicle to visit clients
