What You Will Do

  • Collaborate with multi-disciplinary teams within the Manchester Community Reablement (MCR) pathway.

  • Undertake strengths-based assessments and reviews, focusing on improving function, independence, and quality of life.

  • Develop and implement goal-oriented support plans tailored to each individual's reablement journey.

  • Conduct moving and handling risk assessments, ensuring provision and safe use of necessary equipment and adaptations.

  • Guide and mentor Reablement staff to enhance their goal-setting and support delivery.

  • Offer expert advice on specialist equipment and reablement strategies, ensuring safety and progression for service users.

  • Monitor progress, adjust interventions accordingly, and ensure outcomes are met in alignment with Care Act principles.


Key Requirements

Qualifications:

  • HCPC-registered Occupational Therapist

  • Degree or equivalent qualification in Occupational Therapy

Experience:

  • Proven background in community-based OT, ideally within reablement or intermediate care services

  • Strong knowledge of moving and handling techniques, risk assessment, and equipment prescription

  • Experience working in a multi-agency context with health, social care, and voluntary partners

Skills & Attributes:

  • Excellent communication, documentation, and organisational skills

  • Able to work autonomously and adapt in a fast-paced environment

  • Confident in providing training and mentoring to non-clinical staff

  • Committed to person-centred, outcome-driven practice
