What you will do:
- Deliver a high-quality specialist Clinical Pharmacy Service across community hospital settings, working closely with the Lead Pharmacists for Hertfordshire and Bedded Services.
- Provide support and guidance to Medicine Management Pharmacy Technicians, fostering a collaborative team environment.
- Take charge of individualised patient pharmaceutical care at ward level, ensuring clinical prioritisation on a day-to-day basis.
- Conduct medicines reconciliation and full clinical screening of prescriptions to guarantee the safe and cost-effective use of medicines, aligning with local and national guidelines.
- Facilitate changes in patients' medicine use to enhance safety, adherence, and promote independent use where appropriate.
- Implement medicines management initiatives to reduce drug waste and improve patient flow, ultimately facilitating timely discharges.
Our ideal candidate is a GPhC-registered pharmacist with a valid UK driving licence, as travel between wards may be required.
