What you will do:
- Provide direct support to children and young people with SEND, facilitating their participation in learning and development.
- Carry out assessments and develop tailored plans to ensure the individual needs of young people are met effectively.
- Offer expert advice, guidance, and support to children, young people, their families, and carers across various settings.
- Collaborate with key partner agencies and deliver a coherent approach to support by brokering services from specialist providers.
- Represent the organisation within multi-agency forums to ensure a unified approach in supporting children and young people.
- Develop strong, positive relationships with young people, parents, carers, and partner agencies to encourage integrated support.

Purpose of this role:
The role is pivotal in removing educational barriers for young people with SEND, offering essential guidance and facilitating the support necessary to prepare them for adulthood. You will play an integral part in ensuring that their needs are met in an integrated, coherent, and sustained manner, providing direct support or organising assistance from specialist services.

Requirements:
- Significant experience working with young people aged 0-25, both individually and in groups, across various settings.
- Proven ability to represent the organisation in multi-agency forums and build effective working relationships with young people, partner agencies, parents, and carers.
- Experience in assessing the needs of children or young people and providing pertinent information, advice, and guidance.
- Possess an NVQ 4 or equivalent qualification related to the work.
