Job Description: Band 6 Community Psychiatric Nurse within Adults CMHT

What you will do:
- Provide comprehensive mental health assessments and interventions to service users within the community.
- Collaborate with multi-disciplinary teams to create and implement personalised care plans.
- Offer therapeutic support and guidance to individuals, helping them achieve their recovery goals.
- Conduct home visits and liaise with other healthcare professionals to ensure a coordinated approach to patient care.
- Engage with patients' families and carers, offering them support and education on mental health.
- Maintain accurate and up-to-date clinical documentation and reports.
Requirements:
- You must hold a degree or equivalent qualification in Mental Health Nursing with current registration with the NMC.
- Previous experience as a Mental Health Nurse is essential to thrive and provide excellent care in this role.
