Location: Dudley, UK
Pay Rate: £31.97 per hour
Contract: Locum | 6–18 months
Hours: Full Time | 37 hours per week

Bring your expertise to Dudley as a Social Worker within the Mental Health Hospital Discharge Team. This is an excellent opportunity to play a key role in supporting mental health recovery and delivering safe, timely hospital discharges. You’ll work in close partnership with NHS organisations, including Black Country Health Care Trust, and local authority social work teams to ensure service users transition successfully back into the community.

What You Will Do

• Complete Care Act assessments and make informed recommendations

• Undertake capacity assessments and participate in ward reviews, pre-discharge meetings, and Section 117 reviews where required

• Liaise with housing providers and external support agencies to support timely and effective discharges

• Meet regularly with health discharge coordinators to ensure smooth discharge processes

• Act as a key point of contact for accessing social care support, providing updates and escalating issues when needed

• Maintain effective communication with locality adult social care teams

• Explore respite and step-down options to support service users appropriately

• Attend funding panels to present cases, make recommendations, and support Section 117 funding applications

• Strengthen partnership working between health and social care organisations
