What you will do:
- Conduct thorough assessments of individuals and families to identify their needs and tailor support plans accordingly.
- Develop, implement, and regularly review care and support plans to ensure they meet the evolving needs of service users.
- Safeguard children and vulnerable adults by responding appropriately to any signs of abuse, neglect, or exploitation.

- Provide a range of support, including emotional and practical assistance, to service users and their families in various challenging situations.
- Actively advocate for the rights and desires of individuals, ensuring their perspectives are respected and considered in all decisions.
- Collaborate with a wide array of professionals, including NHS staff, police officers, educators, and housing teams, to deliver comprehensive support packages.
- Make informed referrals to appropriate services and coordinate support to ensure continuity of care for service users.
- Maintain detailed, confidential case records that are up to date and accurate, supporting the delivery of high-quality care.
- Prepare and present professional reports, contributing to case reviews, meetings, and, when necessary, court proceedings.
- Monitor the effectiveness of support plans and adjust them as needed to reflect changes in circumstances or outcomes.
- Manage a varied caseload, prioritising urgent and high-risk cases with efficiency and care.
- Operate in line with all relevant legislation, safeguarding procedures, professional standards, and organisational policies.
- Engage in ongoing professional development through training, supervision, and additional learning activities.
