What you will do
- Perform specialist assessments, rehabilitation, and discharge planning for complex patients.
- Conduct clinical triage of referrals into Intermediate Care services.
- Complete trusted assessor assessments.
- Assess, order, and review specialist equipment.
- Coordinate complex discharge pathways from acute and community settings.
- Manage a specialist caseload and coordinate patient flow.
- Lead case conferences to support safe and timely discharge.
- Provide specialist clinical advice to patients, carers, and professionals.
- Support admission avoidance and facilitate earlier discharge.
- Make recommendations regarding healthcare packages and onward referrals.
- Liaise with multidisciplinary teams, community services, and care providers.
- Maintain accurate clinical records and documentation.
- Support service development, audits, and quality improvement initiatives.
- Mentor, supervise, and appraise junior staff members.
- Deliver training on discharge planning and Intermediate Care pathways.
Requirements
- HCPC Registration: Must be a qualified Physiotherapist or Occupational Therapist with current HCPC registration.
- Relevant Experience: Previous experience in discharge planning and community-based settings is essential.
- Communication Skills: Excellent communication, report-writing, and interpersonal skills.
- IT Proficiency: Good IT skills with the ability to maintain accurate clinical documentation.
- Time Management: Strong organisational, prioritisation, and workload management skills.
- Problem-Solving Ability: Capability to make sound decisions in complex situations.
- Person-Centred Approach: Positive attitude towards working with older adults and supporting their independence.
