What you will do

• Conduct Care Act assessments and reviews for adults transitioning from hospital to community.
• Collaborate with hospital teams to ensure safe and timely discharges.
• Craft person-centred care plans that encourage recovery and maximised independence.
• Evaluate eligibility for adult social care services and propose suitable reablement options.
• Liaise with multidisciplinary teams including occupational therapists, nurses, and community organisations to deliver comprehensive care.
