What you will do

  • Conduct strengths-based assessments of adults' needs.
  • Identify eligible needs and desired outcomes in line with the Care Act 2014.
  • Develop person-centred plans that promote choice, control, independence, and wellbeing.
  • Implement, review, and amend care and support plans based on assessed needs.
  • Manage a varied caseload effectively, prioritising work according to risk and urgency.
  • Maintain accurate and comprehensive case records in accordance with statutory and organisational requirements.
  • Respond appropriately to safeguarding concerns and undertake safeguarding duties where required.
  • Work collaboratively with partner agencies to develop and implement safeguarding plans.
  • Apply the principles of the Mental Capacity Act in assessments, planning, and decision-making.
  • Support individuals who may lack capacity through best-interest decision-making processes.
  • Recognise and respond to carers' needs, promoting resilience and wellbeing.
  • Develop effective working relationships with professionals across health, housing, voluntary, and community sectors.
  • Participate in multidisciplinary meetings and contribute to coordinated care planning.
  • Promote preventative approaches that reduce escalating needs and support independence.
  • Explore and maximise community resources and local support networks.
  • Consider financial implications when developing care plans and adhere to council policies regarding financial assessments.
  • Maintain professional social work standards through supervision, reflective practice, and continuous professional development.
