What you will do
- Conduct comprehensive assessments of adult social care needs under the Care Act 2014, focusing on strengths-based support plans.
- Undertake Mental Capacity Act 2005 assessments and support best-interest decision-making where appropriate.
- Complete Social Circumstances Tribunal reports and act as Social Supervisor for conditionally discharged patients under Sections 37/41 of the Mental Health Act.
- Serve as a Safeguarding Enquiry Officer, working alongside Safeguarding Adults Managers to address safeguarding concerns and manage risk effectively.
- Participate in the duty system on a rota basis, screening referrals, responding to safeguarding concerns, and supporting service users, carers, and professionals.
- Work collaboratively with multidisciplinary teams to facilitate safe and effective hospital discharge planning.
- Support individuals to achieve greater independence, recovery, and positive community outcomes.
- Maintain accurate case records, assessments, and professional documentation in line with statutory requirements.
